Choosing your route and provider

A worksheet for recording what you find while comparing routes and providers for ADHD assessment. It organises information. It does not assess symptoms or recommend a provider. Waits, prices and arrangements change often, so note the date beside anything you're told.

The GP conversation

Your GP may be central to the NHS and Right to Choose referral routes, and may later decide whether to accept Shared Care. Keep this separate from any single provider.

Why this matters Written reasons matter. They tell you exactly what to address, and they're what an ICB or a complaint would need to see.

If using Right to Choose England only

Watch out These are different answers with different consequences. Declining Shared Care with a particular provider later is separate from whether you can be referred there in the first place. Where the legal right to choose applies, the key questions include whether the referral and provider are clinically appropriate and whether the provider holds a qualifying NHS contract for the service required. Practice preference — “the one we use” — is not by itself a reason to override that right. There are exceptions to the legal right to choice, so if you are told you cannot be referred to a provider you have chosen, ask for the exact reason in writing.
